LONGFELLOW-SEWARD HEALTHY SENIORS PROGRAM

Our mission: To enable community seniors to live healthy, independent and socially connected lives.

We serve about 600 seniors annually, through programs such as:

"Nurse is In" / blood pressure clinics at nine community locations * In-home nursing visits, including vision consultations * Monthly Senior Social/Health Talks * Volunteer-provided services such as "Friendly Visitor" volunteers for isolated seniors, help with shopping and transportation * Diabetes Support Group * Art therapy classes * Tai Chi Easy exercise classes * Caregiver support and education * Rapid Screen assessments * Foot / home care services * Information, referrals and advocacy for securing services * Transportation to local medical appointments (limited)

A Longfellow/Seward Healthy Seniors Story:

Peggy is over ninety years old and has lived in Minneapolis all her life.  Healthy Seniors provided support and respite for her while she was taking care of her ailing husband.  Peggy is outgoing and joyful, and enjoys writing poetry and drawing.  We provide two "Friendly Visitor" volunteers who visit Peggy, one of whom plays Scrabble with her.  We also provide her with occasional rides.  She enjoys participating in our Senior Social / Health Talks and our art classes.

Through your donation we are able to build neighborhood and volunteer support systems for seniors that rely on human, one-to-one relationships and services, uniquely designed for each individual's needs.
