Honoring Choices

Honoring Choices' mission is to educate, empower, and inspire individuals to engage in comprehensive advance care planning so their healthcare preferences for the future will be expressed and honored.

Who would speak for you if you couldn’t speak for yourself? Advance Care Planning (ACP) is a process which helps you think about, talk about, and write down your preferences for future health care. While it is not an easy topic to consider, it is important for every adult 18+ to have a health care directive – a written plan for loved ones and healthcare providers to follow – so that your wishes are known in case a severe injury or illness renders you unable to communicate.

And with your help, now through the end of December, your donation will be matched, up to $20,000, thanks to one of our generous granters. 

Honoring Choices Minnesota is committed to helping every adult Minnesotan understand the process of advance care planning. We provide direct support to individuals and tailor training for professionals from various fields - such as healthcare workers, lawyers, social workers, etc. - to skillfully navigate and guide ACP conversations. Additionally, we create and distribute high-quality educational materials such as our health care directive forms in seven languages, terminology FAQ sheets to help aid understanding around key medical terminology, and partner with community organizations to ensure the ACP needs of Minnesotans are addressed. This full-circle approach strengthens the likelihood of having your choices honored when you are unable to express them.
