Who is HealthFinders Collaborative?

Since 2002, HealthFinders Collaborative (HFC), a nonprofit health clinic in Rice County, Minnesota, has led the way in addressing and eliminating health inequities and creating a vision that ensures every person in Rice County feels secure about their healthcare. With three locations in Northfield, Faribault, and Owatonna, HealthFinders is the primary medical and dental office for the patients they serve, providing:

  • Regular dental exams
  • Well child checks
  • Diabetes management
  • Countless other services to individuals and families who would otherwise not have access to care

Patients pay what they can, and generous supporters and community partners help cover the rest. Staff and volunteer healthcare providers, nurses, interpreters, and receptionists, plus support and services from local health system partners and community organizations, make this work possible. The nonprofit collaborates with local Sterling Pharmacy for medical prescriptions.

Meeting Today’s Urgent Needs. No Matter What.

The COVID-19 global pandemic impacted everyone, and HealthFinders has continued to fulfill its mission and ensure health care access for everyone throughout it all. Despite uncertainty and risk, HealthFinders never closed, and, in fact, expanded services to respond to community needs and address barriers to care by:

  • Providing access to free COVID-19 testing, covid vaccines, and monitoring of ill patients
  • Waived prescription copayments for three months
  • Responding to the opiate crisis by expanding MAT (Medication Assisted Treatment) services
  • Ensuring patient access to important public health information by providing culturally appropriate, multilingual resources to address language barriers 
  • Providing assistance and advocacy to anyone who needs help with unemployment or insurance application or accessing community resources
  • Adapting services to keep everyone safe – from offering virtual medical and bilingual mental health visits to emergency dental care and more

Health Happens Everywhere 

HealthFinders’ services are rooted in a holistic, community-based approach to caring for the whole patient, understanding that health happens in homes and communities, not just clinics. HealthFinders makes this happen through:

  • School-based services, such as teen medical/mental health care and dental services for elementary students
  • Community engagement with individuals and groups to raise the voices of the underserved and address community needs
  • Culturally responsive wellness programming focused on nutrition, healthy lifestyles, and diabetes care
  • High-quality, bilingual clinical care, ranging from medical and dental services to medication assistance, insurance enrollment, and community resource connections 
  • Addressing racial and social injustices by offering informative, collaborative, intercultural seminars to promote understanding

Through Give to the Max Day 2022, HealthFinders hopes to raise $15,000, which will make an immediate impact – increasing access to life-changing medical services for ALL through remote, bilingual, and non-barrier care.
