For over 30 years, Catholic Eldercare has provided affordable care and welcoming homes for Twin Cities seniors. Catholic Eldercare offers a caring, community atmosphere for individuals of all faiths, where they receive the support, respect, and spiritual enrichment they need. 

Over the years, we have continued to expand and improve the continuum of care offered in our communities. Recently, we opened our Transitional Care Unit and made our outdoor spaces safe and accessible for all residents by re-landscaping the green space next to historic St. Anthony of Padua Chapel. Most recently,  we've concluded a beautiful renovation of the chapel interior that  improved accessibility there, as well. 

Your contributions helped make all these things possible and will continue to support nurturing of life's ageless spirit  in these areas: 

• Skilled Nursing: Full-time senior services at our nursing home in Northeast Minneapolis, the Albert J. Hofstede Care Center, with 24-hour skilled nursing care.  
• Memory Care: A safe environment where residents with dementia or other memory concerns receive dedicated care from highly trained caregivers. 
• Assisted Living: Assisted living residences help restore independence by taking care of daily concerns that make living alone unfeasible. 
• Transitional Care: After a hospital stay, transitioning back home can be difficult. Having the right support team and services can speed up the healing process, improve outcomes and get you back to health—and back to home. 
• Rehabilitation Therapies: Physical, Occupational and Speech therapies focused on functional needs and overall wellness. 
• Adult Day Services: Provides socialization and activities that engage the whole person such as spiritual growth activities, health monitoring and education, skilled nursing service, medication assistance, bathing services, and full-service barber and hair salon.
• Independent Living: Tenants maximize their independence by enjoying the conveniences of community life with comfortable housing options for those who lead an active life but appreciate the security and convenience of Catholic Eldercare’s community resources.
